Here you see how we have inserted the main hook just to the side
of the pillies  gills and pushing the point up through the head.  This helps to keep the pillie  intact.  The keeper hook is inserted further down to give better bait  presentation and to hold everything in place.  Wrap some bait elastic or cotton  around the whole deal for further security if desired.

Here is a pilchard cut in half.  Try to  make a diagonal cut to expose more flesh and blood juices.  Insert the hooks in  the same manner as above and wrap bait elastic to hold everything  securely.

Next time you go out fishing, try this awesome cocktail  bait
concoction.  Here I have first threaded on a fresh mussel passing the main  hook through several times, then I threaded on a small pilchard tail (just  hooked through once) and then threaded on a strip of squid.  The squid helps  hold the other baits on the hook.  Lastly, use the keeper hook to hold up the  mussel on the shank of the main hook.  Wrapping with bait elastic will ensure  everything stays in place.
